Dr. Persky was privileged to attend and honored to speak at The 6'th Annual Facial Cosmetic Surgery 2010 meeting, an international multi-specialty symposium which gathered over 800 core facial plastic surgery specialists from 23 countries. World leaders in the specialties of Cosmetic Dermatology, Facial Plastic Surgery, Oculoplastic Surgery, and Plastic Surgery gathered in the Las Vegas mid summer desert heat to discuss "hot" topics in their fields.

Dr. Persky shared his nearly 3 year experience with FRACTIONATED CO2 LASER skin rejuvenation (Fraxel repair). Fractionated CO2 laser treatment has become an accepted non-surgical technique of facial rejuvenation.

Dermatology experts Susan Weinkle, MD; Ava Shamban, MD; Wendy Roberts, MD; Hema Sundaram, MD; Phill Werchler, MD; Rebecca Fitzgerald, MD; Mary Lupo, MD; David Goldberg, MD; and Lisa Grunebaum, MD to name a few shared their skin care "pearls" as well as their injection and other rejuvenation techniques.

Every patient should be on a RETINOID (Retinol, Retin-A, Renova, etc.)at night if tolerated for optimal skin tone, color, and texture. Fillers are very popular. Dr. Persky believes that SCULPTRA will become a very popular filler in the next five years. TAZORAC is a wonderful topical for the treatment of acne and pore size.

Non-invasive skin tightening technologies were discussed. Zeltiq looks promising using cold to shrink the fat of love handles and abdominal pooches. Ulthera uses ultra-sound energy to lift the tissues of the forehead, neck, and face. Pelleve` uses radiofrequency (RF) energy for moderate collagen stimulation and lifting of the face. Primaevia uses bipolar radiofrequency to diminish wrinkles. E-Matrix uses sublative RF to rejuvenate darker skin types.

Facelift experts are using fat transfer in nearly all of their lifts to supplement facial volume. Fat transfer to the face is alive and well! The latest Rhinoplasty and Blepharoplasty techniques were shared by world leaders as well.

There are new filling agents that will soon compete with hyaluronic acids such as Juvederm and Restylane. The topical "Botox" Revance looks to be a few years away from hitting the market with promising results on the treatment of "crow's feet" and excessive sweating of the underarms and hands.

That's all the news from Las Vegas, where all of the faculty were great, all of the exhibitors friendly, and all of the children not allowed in the casino (with apologies to Garrison Keillor).

When evaluating new and exciting medical technologies and advances it is essential that we use good science as well as common sense in our evaluations. Unfortunately many highly touted technologies are just that, hype and company generated promotion without proven science or clinical studies to substantiate their claims.

I have just returned from the Facial Cosmetic Surgery 2010 meeting, a meeting well respected for its faculty and its content. Sadly, even at great scientific meetings, valuable time and mental effort is sometimes wasted in separating the truth from the hype.

I have spent the 30 years of my professional life in medicine establishing my reputation, and am not willing to compromise ethical care to my patients based on unethical paid consultant presentations. I will continue to offer my patients only clinically proven, safe, and effective treatments in a caring manner. After all, we should do onto to others as we wish they do unto us.

You also earlier asked about your asymmetrical nose one month after your rhinoplasty. We ask our rhinoplasty patients to avoid full contact sports for at least 3 weeks after rhinoplasty. Those that are playing sports that involve balls are told to be careful not to be hit in their nose with the ball. In your case, it is okay to "have lot's of sex" 2 months after your rhinoplasty, but once again, try to keep the balls from hitting your nose. Good luck on all fronts.

Will You Still Need Me, Will You Still Feed Me When I'm Sixty-Four?

Ever wonder what you might look like when you are older, or more importantly what your prospective mate may look like when they are 64? The Beatles weren't the only ones to ponder that question. What is striking is the loss of volume in the face as it ages. Fortunately, in real life, we can use fillers such as Sculptra, Radiesse, Restylane, Juvederm, and fat to replace volume in the face. The trick is finding the best physician "sculpturer" to accomplish the rejuvenation. Remember, the best plastic surgery is always natural and undectable to fellow shoppers on Rodeo Drive in Beverly Hills. It is okay to "puff" the face naturally, but avoid those Pillsbury Boy aesthetic physicians. Choose your facial cosmetic physician most carefully. Be well.

Zerona Laser, Dr. Persky's Thoughts

> Many of my patients, and many more of you out here in internet-land have asked me what my thoughts are regarding Zerona laser treatments. For the past year I have researched Zerona technology (since it came to market). I remember first seeing advertisements for Zerona at The Aesthetic Show in Las Vegas, July 2009. The ads were the first of their kind, pasted on the mirrors in the restroom outside of the lecture conference halls at Caesar's Palace. This was my introduction to the creative marketing efforts of the company behind Zerona. The company has employed Leeza Gibbons as their spokesperson.

Zerona claims that patients lose 3 1/2 inches to 9 inches (from their waist, hips, and thighs) in 2 weeks with 6 every other day 40 minute sessions (20 minutes on the front, 20 minutes on the back). All patients need to do is lay still and let 5 painless laser lights rotate on their skin (some practioners even treat OVER clothing) like a prom night laser light show.

Unfortunately, the science of lasers does not support the Zerona's claims.

As mom always told us, if it "sounds too good to be true, then it usually is". I am afraid that in my opinion as an expert laser surgeon, Zerona's claims are too good to be true. I really wanted Zerona to be the real deal, and offer it to many satisfied patients. At $2000 to $3000 per treatment series, the financial incentives are very attractive. I cannot ethically and in good conscience offer my patients a treatment that is scientifically unsound.

Zerona's 635nm laser cannot penetrate the skin so it cannot have any effect on fat cells. The electron-microsope studies that have shown "microscopic holes in the fat cell membrane and leakage of fat from the cells" were done with the laser DIRECTLY applied to fat cells underneath the skin, with no skin to penetrate. The loss of "inches and pounds", if any, comes from the manditory Curva niacin supplement drink, diet, and exercise that desperate patients must follow (this explains why there is "loss in areas that were not even treated by the laser). The company claims are that the "laser triggers a response in the body that causes the fat cells to communicate, causing a general effect in the entire body". Recall that the laser cannot penetrate through the skin to be able to affect the fat cells.

There have not been any comparative studies looking at results with the use of Curva supplement, diet, and exercise alone versus with the addition of the laser light. Given that the laser cannot penetrate through the skin to have any effect on fat cells, most likely the results will be the same without patients having to waste their time having the Zerona laser lights rotating on their skin.

As for me, when the time comes for my inches to be lost, I will try the "AB Circle Pro" at $199 plus shipping and handling. Their before and after photos looked more impressive than the Zerona photos. A few yoga sessions, honest exercise, and diet will likely help as well. Pass me that scoop of Baskin-Robbins Rocky Road ice cream please. Be well.

What's Old, New, and Hot in Facial Plastic Surgery 2010? Part I

We traveled to Italy, visiting the Vatican and Michaelango's Sistine Chapel, Bernini's sculptures, and Da Vinci's masterpieces. Michelangelo’s David (1504) in the Uffizi Museum, wow! Wonder what these great masters would be doing if they were facial plastic surgeons today?
They probably would have been in Las Vegas (110 degrees) last week at the Bellagio attending Facial Cosmetic Surgery 2010, a multi-disciplininary meeting of some of today’s master artists in plastic surgery. Many of the world's top leaders in plastic surgery, facial plastic surgery, ophthalmic-plastic surgery, and dermatology shared their knowledge and experience at an incredible meeting.

One of the "hot" topics (full pun intended) this year was fractionated CO2 laser skin rejuvenation. I was privileged to present my nearly 3 year experience with Fraxel repair fractionated CO2 laser. Course chairman Randy Waldman, MD, Ed Cortez, MD, and Waffles Wu, MD put together a faculty that would have kept the great Italian Masters awake with interest (maybe with a few cups of coffee) with their incredible results.

The Secret to Looking Your Best....Puff and Buff (PB)

The study of youthful, beautiful faces reveals two common findings. A youthful face has adequate volume or fullness, and secondly, the skin appears flawless and well hydrated. With aging, we lose volume from our faces through the loss of fat, bone, and muscle. The skin ages due to loss of underlying support as well as from sun damage. Rejuvenation of the face can be achieved by "puffing and buffing" (PB). I originally referred to the treatment as buffing and puffing (BP), but after the BP Gulf oil disaster thought that PB was more appropriate.

PB is the combination of restoring volume to the face with fillers (Sculptra, Juvederm, Restylane, Radiesse, and fat transfer), and improving skin tone, texture, tightness, and color with lasers and other modalities (Fraxel Dual, restore, repair; O2Intraceutical oxygen infusion facials). The treatment is individualized for each patient. We ask patients to bring photographs of when they were younger and felt that they looked their best. By comparing the photos with their current state, a plan is made to restore their youthful appearance, helping them look their best.

While I am on the subject, for those of you who don't already use one, Buff-puffs are an excellent old fashioned technique of exfoliating you skin and keeping it "buffed" inexpensively at home. Clarisonic is also a great at home treatment for the skin. I guess a little self "buffing" at home won't hurt, but please do not try to "puff" at home on your own, unless it is by putting on a few pounds; pass me the cheesecake please. Be well.

Sculptra: The Many Uses in Rejuvenating the Face

Sculptra Aesthetic is FDA approved to treat nasolabial folds (smile lines from the nose to the corner of the lip). Off-label use of Sculptra includes filling hollows in the temple area, building up the upper lateral eyebrow which in turn gives more show to the upper lateral eyelid, adding volume to cheeks and cheek bones, replacing lost volume at the base of the nose and maxilla resulting in elevation and shortening of the upper lip restoring a youthful proportion between the upper and lower lips (1:2 ratio), treating lost volume in the chin, pre-jowl area, and building up the jaw line. Other volume depleted areas of the face can also be treated. A natural lift of the cheeks and jaw line can be achieved in most patients.

It can be used in younger patients who are just beginning to see early signs of aging (loss of volume in the face, early jowls) using a small amount of Sculptra properly placed restores that "no worries face of earlier years". Athletic women and men, as well as those who diet, tend to lose volume in their face which doesn't look as good, healthy, and youthful as the rest of their body. Now dieting and working out does not have to be tempered to keep volume in the face. Sculptra aesthetic can restore volume in the face. Older patients who do not wish to undergo surgical rejuvenation can have a natural rejuvenated appearance with Sculptra aesthetic.

The results are gradual over time. Depending on the individual 1 to 3 (or sometime more) sessions separated by 6 to 10 weeks are used to get "complete" correction. Studies have shown that once completely corrected, 85% of Sculptra Aesthetic patients are still satisfied with their results 26 months later. Though initially slightly more expensive that traditional fillers such as Juvederm, Restylane, and Radiesse, when amortized over the years, the cost of Sculptra aesthetic is much less that those other fillers.

I ask that my patients bring in photos of when they most loved how their face looked in the past. We study their photos together in consultation. After analyzing their photos and face, a customized plan is made to restore their lost volume with Sculptra Aesthetic.

A More Comfortable Restylane, Perlane, or Juvederm Experience: Restylane-L and Juvederm -XC

Now there is a more comfortable solution for patients who are hesitant to start or maintain their facial filler injection treatments because of perceived discomfort during injection.

Both Juvederm-XC as well as Restylane-L and Perlane-L have "numbing" medicine (lidocaine) mixed in with their product. This results is much less discomfort. Patients are noting a much more pleasant "filler" experience. The pre-mixed with lidocaine syringes are noticeably more comfortable than even those syringes that we have manually mixed with lidocaine in the past.

So much for "No pain, no gain"! Be well.

Beauty Tips From The Late Audrey Hepburn

Recently while cleaning out my desk I came across an article with Audrey Hepburn's list of beauty tips. Ms. Hepburn, an internationally acclaimed beauty, was allegedly a carrier for the Belgian Underground Anti-Nazi Movement in WW II as a teen. She learned the importance of a good heart as well as good character early.
Here is her list:

1. For attractive lips, speak words of kindness.
2 For lovely eyes, seek out the good in people.
3. For a slim figure, share your food with the hungry.
4. For beautiful hair, let a child run his or her fingers through it once a day.
5. For poise, walk with the knowledge you'll never walk alone.
6. People, even more than things, have to be restored, renewed, revived, reclaimed, and redeemed; never throw out anybody. Remember, if you ever need a helping hand, you'll find one at the end of your arm. As you grow older, you will discover that you have two hands--one for helping yourself, the other for helping others (by the way, Kirk Douglas in a recent article said that helping others keeps the elderly from becoming depressed).
7. The beauty of a woman is not in the clothes she wears, the figure that she carries, or the way she combs her hair. The beauty of a woman is seen in her eyes, because that is the doorway to her heart, the place where love resides. True beauty in a woman is reflected in her soul. It is the caring that she lovingly gives, the passion that she shows. And the beauty of a woman only grows with passing years.

Thank you for your wisdom Audrey, you were physically beautiful, but your soul will remain eternally beautiful.

Award winning Emmy writer/director/producer and professional baseball announcer, Ken Levine, wrote a beautiful piece about the late Walter Cronkite. Here is what Ken said:

"He told us that President Kennedy had died.
He told us that we had landed a man on the moon.
He walked us through national conventions.
No one was elected President until he said so.
The Viet Nam War ended when he said it was time to go.
He reported on the bombings of London.
And the Olympic skiers in Squaw Valley.
There were never any articles about his hairstyle.
We never knew whether he was Republican or Democrat.
He reassured us in times of national crisis.
Celebrated our triumphs and called us on our shortcomings.
He had no agendas.
He wasn’t folksy or perky.
He was the voice of reason.
The conscience of the country.
He was just… Uncle Walter. The most trusted man in America. And now that voice has been silenced. We’ve lost the last true journalist the broadcast industry will ever see. Just how monumental is his passing?
Name me one person, anybody, who could be called the most trusted man in America now. I can’t think of one either.

The Beauty of the 2010 Vancouver Winter Olympics

Sculptra Aesthetic 2010; Dr. Persky's Thoughts

Sculptra Aesthetic (Dermik Lab., Sanofi-Aventis) is an exciting, new facial injectable treatment. Sculptra is injectable Poly-L-lactic acid (PLLA). PLLA is what absorbable suture material is made of. Sculptra has been used cosmetically outside the U.S. since 1999 and is a very popular procedure in Europe and Canada.

Dr. Persky has been following the use and development of Sculptra since its introduction to Europe in 1999. As with all new products and treatments related to facial plastic and reconstructive surgery, Dr. Persky is extremely conservative and believes that it is important that the treatment be proven not only effective, but more importantly safe. The Hippocratic Oath admonishes, "Physician, first do no harm!"

Early Sculptra treatments were very effective at replacing lost facial volume, so much so that in 2004 Sculptra was FDA approved for use in volume restoration from fat loss (facial lipoatrophy) for HIV patients. Some plastic surgeons and dermatologists began using Sculptra off-label to treat non-HIV lipoatrophy patients for cosmetic volume replacement. There were troublesome nodules and granulomas (lumps and bumps) during the early years of treatment with the product before techniques were improved.

In the past few years, physicians have learned how to minimize the occurrence of nodules with Sculptra. It is now injected in a much more dilute fashion. Patients massage the injected areas for 5 minutes, 5 times a day for 5 days. Injection techniques now avoid injecting through muscles that previously caused nodules. Superficial injections are now avoided. With a much greater safety profile, Dr. Persky now feels very comfortable treating his patients with Sculptra.

At The South Beach Symposium in February 2009, Dr. Persky met with the world leaders in Sculptra use, including Danny Vlleggar, MD. Impressive and natural before and after photos of patients treated with Sculptra, along with the much improved safety profile piqued Dr. Persky's interest in the product. What ultimately convinced Dr. Persky that Sculptra was going to be the filler of the future was seeing and speaking with 3 relatively young, beautiful, intelligent, well known female U.S. dermatologists who have had Sculptra injected into themselves as their facial volume filler of choice. If it is good enough for those experts to use on their world famous faces, then that was good enough for Dr. Persky to begin using Sculptra.

In July of 2009, the FDA approved Sculptra for the cosmetic indications of volume deficiency and facial wrinkles. Once it was FDA approved, Dr. Persky began using Sculptra to contour and sculpt faces.

The beauty of Sculptra is that is works by stimulating natural collagen production. John Joseph, MD of Beverly Hills says, "It is like putting a grain of sand into an oyster to form a pearl". I like to think of Sculptra as wonderful "seeds" that are spread where facial structure needs volume to restore youth.

Sculptra Aesthetic is different from all other filler products. Sculptra is injected as a very dilute cloudy solution that is mostly water and local anesthetic. Once injected, the patient can see a "preview" of what they will ultimately look like after their complete correction. Within hours to days, the fluid is reabsorbed by the body, and the treated patient’s appearance returns what it looked like before the injections, with possibly a new bruise or two. Over the next 6 to 8 weeks as collagen production occurs, the patient's face subtly and gradually establishes a foundation for the correction. Re-evaluation and a second injection session are then carried out. Most patients are treated over 3 sessions with varying amounts of Sculptra depending on their individual need and individual response. At full correction, the result is natural and has been shown to last 25 months and longer. As aging continues, touch-up treatments restore lost volume.

Sculptra gives physicians the ability to treat faces after analyzing the skin, muscle, fat, and bone; determining what needs to be replaced, and then using aesthetic injection skills to achieve facial rejuvenation. Patients with early signs of aging from volume loss in their 30's and 40's are great candidates for Sculptra to correct early "jowling" and loss of volume in the cheeks. In older patients, Sculptra can be used beautifully to restore volume in hollow temples, jaw lines, chins, and to shorten elongated upper lips, restoring the proportions of youth to the face. By restoring volume at the base of the nose, the nasal tip is elevated, mimicking rhinoplasty in some patients. Other than in the nasolabial folds, all of these cosmetic uses are "off-label."

Patient satisfaction studies have shown 84.9% satisfaction at 13 months and 80.0% satisfaction at 2 years. Sculptra should not be used in patients with a history of keloid formation or hypertrophic scarring, or in those patients allergic to absorbable stitches.

Dr. Persky is very excited about having Sculptra as a new powerful facial filling and contouring agent to go along with traditional fillers and fat. As with any treatment or product, it behooves the patient to do their homework and choose their injecting physician most carefully. It takes a combination of proper Sculptra technique with an aesthetic sense of beauty and proportion to help patients gradually look natural without feeling different or like someone else.

Rhinoplasty, What to Expect at Dr. Persky's Office

Rhinoplasty is a permanent surgical procedure, so the results should last your lifetime. The most important aspect of your rhinoplasty is choosing an experienced and skilled rhinoplasty specialist whom you can trust with your nose. Rhinoplasty is considered the most difficult of all facial plastic surgical procedures.

Everything about your proposed surgery should be explained to you by your surgeon at your consultation and again at your pre-operative appointment. You should have pre-operative lab work and surgical clearance from an internist if indicated.
On the day of surgery at our office, your anesthesiologist and nurse will ask you a few questions before taking you into the operating room suite. Once you lay down on the bed, you are made comfortable by the nursing staff with pillows and warm blankets. Once the monitors for your heart, blood pressure, and oxygen level are in place, the only uncomfortable thing that you feel is the small amount of numbing medicine that the anesthesiologist injects next to a vein in you hand. The I.V. is then placed painlessly. At this point the anesthesiologist gives a little medicine in the IV that makes you feel very good ("Just Say NO to Drugs, except in the operating room), then we ask that you think of your favorite place with your favorite person, and you are off to pleasant dreams and a restful sleep. Our anesthetists usually use a medication called Diprovin (made famous by Michael Jackson, but in our center only board certified anesthesiologists administer the medication with proper monitoring) and once you are asleep they then place a laryngeal mask (LMA) to facilitate administraton of oxygen. The LMA also protects the airway and stomach from any fluid. Depending on what needed to be done to your nose, your operation may last anywhere from 1 to 3 1/2 hours. I prefer to not use post-operative "packing" into the nose. If you ask patients who have had rhinoplasty,"What was the worst thing about your rhinoplasty?", if they had packing in their nose for a few days, they will say, "The packing!"

Next thing you know, you are waking up in the recovery room with tape and splint on your nose. After about one hour, once you are fully awake, you are allowed to go home. Once you are home, we recommend that patients get into bed with there head elevated up on 3 to 4 pillows so that their nose is elevated well above their heart. Baggies of frozen peas are kept on the bridge of the nose as tolerated. Surprisingly, our patients seldom complain of pain, but rather a stuffy nose or sinus headache type of pressure that is usually relieved with Tylenol.

Patients return to the office two days after surgery to have their nose cleaned of any mucus or dried blood, the nasal passages are suctioned so that breathing is a little better. The nose will become congested again for a few days. On the fifth day, my favorite, the tape and splint are removed and the new nose is unveiled. Swelling and bruising continue to resolve over the next few days so that patients are quite presentable between 7 to 14 days after surgery. Physical activity is limited for the first two to three weeks after surgery. The appearance of the nose should continue to improve over the next 6 to 12 months. We then see our patients at one week, one month, three months, six months, one year, and then once a year as needed.

All patients should be aware that there is usually between a 3% to 10% chance of revision surgery after initial rhinoplasty.

"A 24-year-old woman today was saved from death by a rocket in the most peculiar fashion. No, not by a handsome Israeli soldier-man sweeping her off her feet (although I’m sure that she, along with many other women, wouldn’t mind), but another matter of the heart.

According to a hospital in Netanya, this woman had rocket shrapnel in her chest, which was inches from her heart. However, they were caught… in her breast implants.

Yes, breast implants.

I don’t know if this goes into support of the plastic surgery that tends to run rampant in the Jewish community. But who would have thought breast implants could produce miracles? According to the doctors at the hospital, if she didn’t have them, she would have never survived.

Well, um… ookay. All right, carry on!"

Hot News! Heat: A New Cause of Premature Aging

Los Angeles dermatologist, Dr. Jessica Wu reports that there is a hot new reason for aging of the skin, heat. Here is her report:

"Beware of your heater and fireplace!

We’ve all heard that the sun’s UV rays can damage the skin, leading to wrinkles, sun spots, and premature aging. But recent research shows that heat can also damage your skin in much the same way. Chronic heat exposure (like from sitting too close to a fireplace, or using a hot water bottle continually) can cause a condition called “erythema ab igne,” which produces mottled, red-brown patches on the exposed skin. At first, the blotchy discoloration can fade when the skin is protected from the heat; however, it can eventually become permanent if the skin is repeatedly exposed to high temperatures.

New studies show that heat can damage your skin even before you see this type of discoloration. Exposure to heat has been shown to destroy collagen and elastic fibers in the skin, so over time, high temperatures can lead to thinning of the skin and eventually premature wrinkling. Heat, just like UV rays, has also been shown to stimulate angiogenesis (new blood vessel growth), which can lead to “broken” capillary veins, red blotches, and uneven pigmentation. Lastly, heat has been shown to generate “free radicals,” which are particles that damage collagen and DNA in your skin.

Based on this new information, scientists are now suggesting that heat may cause aging in human skin. Therefore, in addition to sunscreen to block the effects of UV rays, it may also be important to protect your skin from heat. So what should you do?
Avoid sitting directly in front of the heater or fireplace for prolonged periods of time. If you notice your skin becoming red, turn down the heat or walk away to cool down. If you use a small space heater near your desk or chair, change its position so it doesn’t continually hit the same part of your skin."

Facial Plastic Surgery State of the Art 2010; Dr. Persky's Thoughts

The art of facial plastic has continued its constant evolution over the past 5 years. Whether due to innovative new techniques or "the economy", there has been a marked movement away from surgical procedures and towards non-invasive treatments. The most popular procedures in 2010 are fillers, Botox, lasers and skin tightening treatments.

The emphasis today is on replacing lost volume in the face, and restoring the skin surface to it's best possible tone, texture, and color. There is still a place for well done facial cosmetic surgeries; certainly rhinoplasty will never be replaced by non-invasive techniques, despite the buzz about "the non-sugical nose job". Likewise, a well performed neck/face lift will always have its place in those patients with too much excessive skin.

The mainstay of todays facial rejuvenation is volume replacement. With aging, underlying support of our skin is diminished as muscle, bone, and fat is reabsorbed. The overlying skin covering has a smaller scaffolding for support, resulting in new folds, jowls, and wrinkles.

The second essential component of today's facial rejuvenation is tightening of the surface skin along with improvement of skin color, tone, and texture. Fractional CO2 lasers such as Fraxel repair are used to "buff" the skin to it's maximum potential. In addition to the above mentioned benefits, CO2 laser also stimulates new collagen production over 3 to 6 months which increases overall facial volume. Less aggressive lasers such as Fraxel restore and Dual work on fine lines and brown age spots.

Sculptra, Restylane, Juvederm, Radiesse, and fat are the commonly used volume replenishers. Sculptra is unique, working by stimulating new collagen production. Sculptra is excellent for facial contouring, and is the longest lasting product. Once "full correction" of volume and contour is achieved, over 85% of patients are satisfied 3 years later. Restylane is used for filling the hollow areas under the eyelid, blending the lower lid "bags" with the cheeks. Juvederm and Radiesse are temporary fillers that vary in longevity from 9 to 12 months.

Botox and Dysport are neuromodulators that weaken the muscles of expression responsible for creating forhead lines, crow's feet, and laugh lines. They can also be used to tighten neck muscles, improve the jawline, and turn the corners of the mouth upward to create a happier appearance.

Once volume has been replaced and the skin has been rejuvenated, it is important to continue to keep the skin as healthy as possible. The Secret of Youthful Skin 2010, Dr. Persky's Thoughts

Our family spent a few days this spring in Sun Valley, Idaho. The time away from my practice gave me the inspiration to share my thoughts about "The Secret of Youthful Skin" with you. First a little about Sun Valley and its charming ski town Ketchum, Idaho.

Sun Valley takes some time to get to, which has made it attractive to the "Hollywood In-Crowd” since it opened as America's first ski resort in the late 1930's. Early on it took planes, trains, and automobiles to get to Shangri-la, as it was nick-named. Nowadays, just a flight into Boise, Idaho followed by a 130 mile drive (or a direct flight into an airport that may or may not be open depending on weather conditions) will get you there.

Ernest "Papa" Hemingway made Ketchum one of his three homes along with his others in Cuba, and Key West, Florida. He spent his mornings writing, and afternoons fishing and hunting. He loved spending time with actor Gary Cooper, making somewhat of an odd couple. Hemingway took his life (7/2/60) in Ketchum after electro-convulsive shock therapy (for a depressive disorder) caused him to lose his long-term memory; okay for a goldfish, not okay for a prodigious writer. He is buried in Ketchum.

The Kennedys, Lucille Ball, the Schwarzenegger’s, most of old Hollywood, and many Olympic winning ice skaters (Hamill, Flemming, Kerrigan) and skiers (Killy, Stein-Ericksen) have stayed at the Sun Valley Resort. Comedian and social commenter Dennis Miller was at Ciro's (quaint, informal Italian with killer breadsticks...do not visit if you are observing Passover as the temptation may be too great) our first evening in Sun Valley. Though we were very much wanting to hear Mr. Miller's opinion of Obama care, and the latest of addictions, Tiger and Jesse James' "sex" addiction, we respected Dennis and his family's privacy.

Many of the kind and friendly folk in Sun Valley sport serious unabashedly tanned skin. Which finally brings us to the first hint to the secret of youthful skin...pass me the sun block please. The one product that does the most for your skin is an excellent sun block. In our practice we recommend Elta-MD, Ti-Silc, and SkinMedica sun block.

Besides sun block, growth factors, anti-oxidants, peptides, and skin lightening products aide in anti-aging.

Reality? Camp Ronald McDonald

What is reality? We hear so much about "reality" TV. In fact today I was asked by a news journalist to comment about reality TV's Heidi Montag's recent most publicized multiple marathon plastic surgeries. Is it common to video surgeries (her's were filmed) for possible use in a future "reality" TV show? Not "common", and certainly NOT reality!

Later, the same day, I met 10 year old Grayson's mom. She told me about her real life hero. Grayson was diagnosed at age 6 with an inoperable brain tumor. Sorry Heidi, but this IS reality!!!!!

Grayson's dream is to be among the top fundraising teams in this Sunday, April 11'th "Walk for Kids 2010" at Universal Studios backlot (as well as at Victoria Gardens, Rancho Cucmonga and Honda Center, Anaheim). Grayson will be walking 5K, no easy task for him.

In Grayson's words, "My name is Grayson, and I am raising money for Camp Ronald McDonald by walking 5K and getting sponsors to participate. I love going to Camp Ronald McDonald and want to make sure all kids have the same opportunity. you can read my story at www.amazinggray.org"

Fraxel Repair, Vogue Magazine Article, "A Facelift, Revisited", Dr. Persky's Thoughts

Nearly twenty years after recounting her first foray under the knife in the pages of Vogue, author Joan Gage takes stock of what still looks great, what needs a little modern intervention (Fraxel repair), and how times have changed.

Based on my personal experience with Fraxel repair (similar to my esteemed colleague in Massachusetts, Min S. Ahn, M.D. who is the treating facial plastic surgeon featured in this article) since it was unveiled in January 2008, I agree with everything in the article except Daniel C. Baker, M.D.'s opinion that "laser procedures are less enduring and more expensive, in the long run, than surgery."

As the author, Ms. Gage concludes, "But now, a year post-Fraxel (repair), my skin is still free of discoloration and has fewer and less obvious fine lines; the pull lines on my jaw are entirely gone, and the eyelid scars (from previous blepharoplasty) are almost invisible....All in all, I'm pleased with the result which has left my husband (who's a year older than I am) gallantly smiling when he gets comments about his "much younger" wife."

Fraxel repair is an excellent treatment for patients who have had facelift surgery in the past, and do not wish to undergo surgery again.

Heidi Montag Multiple Plastic Surgeries...Love is the Answer

Plastic surgeons are frequently asked what they think about 23 year old Heidi Montag's multiple cosmetic surgeries performed last winter by Beverly Hills plastic surgeon Garth Fisher. The best answer to that question was this weeks episode of the reality TV show that launched Heidi to "reality fame", "The Hills" (April 27,2010; http://www.mtv.com/videos/the-hills-season-6-ep-1-put-on-a-happy-face/1637514/playlist.jhtml )

Heidi returns to her family's home in beautiful Crested Butte to see her Mom for the first time since her surgeries. Those surgeries included an eye brow lift, chin reduction, revision rhinoplasty, Botox injections, fat injections to lips, cheeks, and nasolabial folds, otoplasty, breast augmentation, and liposuction of the abdomen, waist, thighs and back...whew!

Heidi’s mother, sister, and step-father disapprove of her "plastic" look and are appalled that she risked her life to have more than 12 hours of elective cosmetic surgery, filmed by the way for yet another possible future reality TV show. Her mother implies that Heidi looked better before her "Octo-Plus" surgeries. Heidi shares her pain from the multiple procedures (unable to chew hamburger without severe pain, probably from her chin reduction), and her childhood insecurities, and the "L.A.-Hollywood 'beauty is everything' spotlight" that led to her obsession with plastic surgery. An obsession that may border on Dysmorphic Body Disorder (DBD).

The big question here is why would a young, beautiful 23 year-old voluntarily submit herself to such drastic body changes in one sitting? Most questions like this can be simply answered by The Beatles song "Love is the Answer". When we don't feel true love, we look for it in "all the wrong places". In Heidi's case, it was obsessing that looking as good or better than the "Barbies" of Beverly Hills, Malibu, and Hollywood, she would find happiness. It is apparent that she has instead found profound sadness. Her sadness is compounded after going through all of these surgeries as well as their long, painful recovery only to meet disapproval from the one person she craves love from the most, her mother.

There are surgical indications for plastic surgical procedures in young patients. A nose too big, breasts too large causing back problems, ears protruding to comments like, "Hey, your cab's doors are open" are all legitimate reasons to undergo an elective cosmetic procedure. Expecting the surgery to "change" one's life is unrealistic. You will not be loved any greater; you will not be more popular or happy; it will not get you that dream job or mate.

So what is the moral of the story, what lessons are to be learned? Best to follow the sage lyrics of Christina Aguilera's "Beautiful"(http://www.youtube.com/watch?v=eAfyFTzZDMM). Know that you are truly "beautiful no matter what they say". Be grateful for what you have. Be careful what you wish for, it may not be the answer. Love is the answer.

Heidi Montag's Breasts, Multiple Surgeries, Dr. Persky in Orange County Register

Dr. Persky was asked to comment on whether Heidi Montag's recent multiple surgeries and the filming of the surgeries for the use in a future "reality" TV show was common.

That’s the ‘breast’ that I can do in answering the breast questions.”

“As far as the surgery video, it is not unusual to video celebrity surgery for ‘reality’ television, but as the old wise man in the pizza parlor said loudly to his equally hard of hearing friends when a particularly steamy scene of MTV’s ‘Jersey Shore’ was on the flat screen, ‘Reality? Dr. Persky Tests New Fraxel Dual Laser System

Two weeks ago Dr. Persky treated our first two patients with the newest Fraxel laser, the Fraxel Dual Laser System. Both patients already have fabulous "Wow!" results.

Using a new wavelength and Thulium laser, the new Fraxel restore Dual Laser System is set to take fractional resurfacing to a new level. The laser is able to not only restore the skin on the face, but on large portions of the body as well. It treats sun damaged superficial skin and in a single quick and pain free office treatment greatly diminishes brown pigmented skin lesions. Both patients experienced no pain, 2 days of redness and minimal swelling followed by about 4 to 5 days of a thin peel. One patient went to work the next day while the other said definitely stay home for a day or two. At 10 days both patients commented how clear their skin appeared, and the most common reaction from friends and family was that they "had make-up on" when they did not. My initial reaction was the same, I actually asked both if they were wearing make up. There was also a noticeable improvement in volume in the face. Both patients have commented that they want the treatment on their arms, legs, and chest as well.

The machine boasts several advantages, including greater ease of use for the physician and a shorter treatment time. Solta Medical (Hayward, Ca) states that the machine will allow greater comfort for the patient and faster results. Fractional resurfacing is a procedure that has been largely limited to the face so the addition of an effective superficial treatment for the body is welcome.

The Dual Laser will be used to complement the other Fraxel® family of products, which includes the Fraxel restore laser system, the Fraxel repair laser system and the Fraxel refine laser system, which have swiftly become the cosmetic industry standard for fractional aesthetic skin rejuvenation treatment. Downtime is minimal involving 2 to 3 days of redness and swelling followed by another 4 to 7 days of peeling (like after one of those early summer sun burns when we were sun worshipping teens). The face, neck, chest, hands, and any other area of the body can be treated. The laser is 25% faster than previous technologies.

Patients comment that their skin looks and feels rosy and fresh, and that even fine wrinkles are improved. One of our most recent patients who had previous experience with multiple series of IPL (Intense Pulsed Light) laser treatments remarked, "The Dual is great, it is like IPL on Steroids!"
