I have been getting so many comments about my skin and face from complete strangers. Everywhere I go people make comments about my skin. It is amazing.

Today was very interesting. I was at the gym. I few weeks ago I had carried these weights over to someone that wanted to use them and must of commented that I was going to be at a convention or he must of heard me. Anyway, he came up to me and asked me how the convention went and wanted to know what type of convention. He thought I was in the SKIN BUSINESS!! He wanted to know what I was doing and what I was using on my skin as he was looking for a distributor from like japan to buy direct or something like that. He is 72 years old. I couldn't tell him what all I had done. Kept it simple, power peels, facials and eating lots of fish, drinking lots of water.

Another customer of mine in San Juan Capristrano pulled me aside and said, "ok, what you doing?? I told her and now she wants to do it. She lives too far away tho.

I guess it is noticeable. It will be interesting this weekend as I am going home to see my family. I am sure I will be asked if I have had a face lift!! I haven't seen them in a year. I can already hear the comments and I surely won't be telling anyone there anything but what I said above. Mumm is the word

You have to laugh..

Just sharing.
T"

"Mumm is the word" is what the majority of my celebrity and non-celebrity patients desire. They want their results to look natural, and don't particularily want to share their photos with the world on my website.

So compared to many other cosmetic physicians, you may not find as many testimonials or before and after photos, but what I can promise you is that I will do my best to keep you looking your best....confidentially.

By the way, this patient had an upper eye lid skin only blepharoplasty, and two sessions of Sculptra injections which restored volume and a more youthful shape to her face.

Be well.

Richard Fitzpatrick, MD (Carlsbad) presented his results using the Zeltiq device. Zeltiq, developed at Mass. GeneralHospital by Rox Anderson, MD, appears to be a great procedure for spot reduction of small fat bulges localized in the abdomen and "love handles" that are resistant to exercise. The transducer is applied to the offending area; suction pulls the skin and fat into the transducer where the fat is chilled for about one hour without damaging the overlying skin.

Dr. Fitzpatrick stated that Zeltiq is virtually risk free without downtime, and that his patients are extremely happy. There have been no significant adverse events, but 50% of patients have significant bruising, some have a temporary alteration in skin sensation, and transient pain. Chris Zachary, MD (Irvine, CA) commented that there have been a few cases of severe pain, and that experts differ on whether it works in all cases. Like all technologies, proper patient selection as well as realistic expectations is essential.

Dr. Persky is currently looking into adding Zeltiq to his practice. Stay tuned, in the meantime keep working out and eating a healthy diet. Be well.

Andrew Erikson wrote a very good piece today published in Paul Callaway's blog about fractionated CO2, with an excellent guide on how to choose your treating physician. Mr. Erikson stresses the importance of choosing the most experienced and skilled laser physician with the particular device.

In other words, once again, "It's not the Arrow, it's the Indian", or in this case, it's not the laser, it's the treating physician (although some lasers are much better and more effective than others).

The article was so informative for prospective patients that I have reproduced it here, with all credit to Andrew Erikson and Paul Callaway:

Selecting the Best Laser Specialist, New Fractional CO2 Laser Resurfacing Facts

There are so many lasers out there now and everyone claims to have the best and most innovative technique to help your skin.
What is the Truth?

Well, according to most physicians and industry experts, there has yet to be a laser that has emerged as the leader in the fractional co2 laser resurfacing arena. The hottest and most popular CO2 devices are the: SmartXide Dot Co2 laser,Ellipse Juvia Fractional Co2 Laser, Reliant Fraxel Re:pair Co2 Laser, and the Lumenis Active FX Fractional Co2 laser. Each of these
lasers differ slightly from the others but they all can provide great results. Physicians continue to argue and debate over the efficacy of their device but few of them will admit that much of the
difference lies in their hands. The outcome of any device is largely dependent upon how skilled the physician is with their device.

It is so important that patients see Fractional specialists who have a proven track record of great results with the latest technology. Odds are, if your physician is well versed in the other laser technologies such as Fraxel or Traditional Co2 resurfacing, then they should be able to perform the fractional laser without any issues. You still have to do your homework and have realistic expectations of the results you can expect to see. You should do your research on the laser he or she will be using and more importantly do your research on the person who will be doing the procedure.

How many of these procedures have you done?

Do you have before and post op pictures from this device?

How long have you been using this device?

Why did you select this device over the others available?

How long have you been performing laser procedures?

Have you written any papers on laser resurfacing or the different laser devices?

What is the difference between Fraxel and Fractional Co2 Laser?

If you do not know the answer these questions, you should visit http://Fractionalc02.com to learn the answers to these questions and much more. You can also watch video demonstrations of the various laser procedures available, view before and after photos, and read articles by top Dermatologists and Plastic Surgeons about this new procedure. You can also visit to learn about the numerous fractional co2 lasers currently on the market.

These are just some initial questions that you can ask to gain a comfort level with the prospective provider. You want to make sure that you have a good comfort level with the practitioner and do not feel like he/she is just trying to sell you something. Remember that there is no BEST LASER but there are the best settings for the results you are after. Many of the laser devices can provide the same outcome but they MUST be in qualified hands to do so.

Andrew Eriksen – Patient Advocate
dermdesignseo@gmail.com

Former Plastic Surgery & Cosmetic Dermatology Administrator and Marketing Director – Managed Laser Demonstrations for 2008 Florida Society of Dermatologic Surgeon Meeting

beautiful and talented Ms. Julianne Moore on the cover) has a
comprehensive article about the use of facial fillers to restore volume
in the face. Well written by Janet Carlson, the article is entitled
"Speaking Volume".

Five highlights of the piece include:

1. "A
filler session will make you look 7.3 years younger on average,
according to a study of ten female patients aged 42 to 59 that published
this March in Dermatological Surgery".

2. "We can confidently report that if
nearly any aspect of your maturing face bothers you enough, you can with
relatively little risk feel safe in giving filler, as they say, a
shot".

3. "What most doctors talk about these days more than
anything else is (facial) volume".

4. A comprehensive list of
"What's in the Syringe?" which lists the fillers available in the U.S.
today along with their lasting power, pros, cons, and costs. Emphasized
is the fact that "The best use of fillers is, however, the work you
don't see." Like everything else that we do in plastic surgery, the best
work is not detectable, obvious, or over done. Lips or breasts should
not walk into the room before their owner!

5. “How to Choose a
Doctor”...the key factor determining success is your doctor's artistry.
The saying "It's not the brush and paint; it's the artist" has been
altered for physician injectors, "It's not the filler (filler material);
it's the filler (physician)". As I advise patients daily, make sure to
choose your treating physician most carefully!

Dr. Persky offers
all of the fillers mentioned in the article. These fillers provide the
"Puff" of Dr. Persky's "Puff and Buff" philosophy of facial
rejuvenation. Volume "puffs" the face while superficial laser
treatments, peels, facials, and excellent skin care programs provide the
"Buff" of the skin. Together these two strategies provide much greater
results than either individually.

After you have your fillers,
make sure that you see Julianne Moore in her current release, "The Kids
Are Okay", an excellent film. Be well.

See web reference for more on "Puff and Buff" http://www.drpersky.com/blog/archives/236-The-Secret-to-Looking-Your-Best....Puff-and-Buff-PB.html

Be patient and you should see your profile improve week by week over the year. If your nose had the perfect desired profile curve at this point, you might be happy for a year or two, but in 5 to 10 years your nose will have "shrunk-wrapped" into a obviously "done-rhinoplasty" nose. Listen to your rhinoplasty surgeon. He/she was there, felt and sculpted your nose, and saw it before the swelling occurred. It looks as if you are a beautiful woman, and that you will have a beautiful nose and profile. Please let us know how things go over the next few months.

If you are an old time TV fan, there was an episode of "The Danny Thomas" show where Danny (a large Lebanese profile nose) decides to get a rhinoplasty against his family's wishes. He goes ahead with the surgery anyway, no one in the family speaks to him for the week that the cast is on. Danny realizes that he made a mistake and wishes that he had not done the operation, it wasn't worth losing his identity with his family. On the day the surgeon takes off the splint, his nose is exactly the same. The surgeon told him that he knew that he really didn't need to change his nose to change his life, so the surgeon only put the splint on his nose.

Danny was happy, his family was happy, and you too should be happy once your nose settles. There is nothing to do at this point anyway for at least 6 to 12 months.

Good luck and be well.

Aol's Stylelist ran an article today by Grace Gold entitled "Silicone Injections Kill 22 Year-Old Woman; Experts Say Tragic Trend is Increasing in Hispanic Communities." I have reprinted the extremely well written and informative article here as it emphasizes the importance of choosing your treating physician most carefully. It can be a life or death decision.

Alejandra Viveros and Guadalupe Viveros'
mug shots from a previous arrest. Photo: Los Angeles Police Department

"The case of a 22 year-old California woman who died this weekend
after receiving silicone injections to the buttocks is just the latest
tragedy in a string of illegal cosmetic surgeries that are dominating
Hispanic and Brazilian communities, experts say.
Mayra Lissette Contreras is thought to have suffered respiratory failure
after alleged "silicone sisters" Guadalupe Viveros and Alejandra
Viveros injected large amounts of what they said was silicone into the
young woman's buttocks for a fuller shape.

While the sisters claimed to be doctors back in their native country
of Mexico, they didn't have a license to practice medicine in the
United States. The injections were done in their home, and they
reportedly advertised butt augmentation services in flyers that were
handed out in public parking lots.

The duo had already been under investigation by local law
enforcement after other victims went to police with complaints of being
sickened after receiving silicone injections. They were both arrested on
June 21st, but released after posting a $20,000 bail -- and have since
fled.

Neighbors in the Los Angeles town of Sylmar suspect the two have most
likely escaped back to Mexico.

Experts say the red flags were glaringly obvious.

"This poor woman may have been the victim of her own ignorance. Common
sense should tell you that something is not right when you're having a
medical procedure in some one's home," says Beverly Hills plastic
surgeon Dr. Leslie H. Stevens. "A more likely scenario is that the
victim knew what she was doing and unfortunately was willing to take her
chances."

Silicone is most well-known for its use in breast implants, where
the FDA-approved medical grade material is safely encased in a closed
shell.

Plastic surgeons stress that silicone should never be injected freely
into the body, as it often solidifies into rock-like lumps. It can also
be mistakenly injected into a vein that would allow the silicone to
rapidly travel to and block the lungs, which is what is believed to have
happened to Contreras, pending an autopsy.

There is one FDA-approved exception; a silicone oil called Silikon
1000 has been deemed safe for eye fluid replacement -- but is not used
for cosmetic purposes. Some plastic surgeons do use tiny silicone oil
droplets off-label as facial injections to prop up pitted acne scars and
to flatten wrinkles, but former president of The American Society of
Plastic Surgeons Dr. James H. Wells says he doesn't even believe droplet
injections are safe.

"I think people should be very, very wary of silicone injections of any
kind. There's still some physicians who think facial injections are
safe, but I'm not convinced," Dr. Wells tells StyleList.

The evolution of Priscilla Presley's
Face: Presley in 1999, 2004 and 2009 Photos: Getty Images |
jpistudios.com

In fact, it was bad silicone injections that resulted in Priscilla
Presley's battle with disfiguring facial lumps and an ensuing lawsuit
the celebrity took out against Daniel Serrano, whom she had hired for
'injection parties' with her Beverly Hills celeb friends. The charming
South American 'doctor' turned out to have no medical license -- and was
found to have injected Presley with the same cheap car-grade silicone
that is used by mechanics.

Recent silicone tragedies have Miss Argentina Solange Magnano
dominated the Latin American and South American communities.

Solange Magnano on the runway in May, 2009. Photo:
EPA/ZUMApress.com

The death of former Miss Argentina Solange Magnano this past
December happened in much the same way as Contreras. Silicone injections
to the 38 year-old's buttocks caused a blood clot to lodge in the
model's lungs, and a three-day fight for her life ensued. Magnano
finally succumbed to a pulmonary embolism, leaving behind two eight
year-old twins.

Experts say it's the cultural desire for a curvier figure that draws
these patients in for buttock procedures.

"Patients tend to be Hispanic and Latin American. This is because a
fuller, more voluptuous "J-Lo" butt is revered in their culture. In the
American culture, a more toned, slender buttock is considered the
ideal," says New York plastic surgeon Dr. Jennifer Walden.

In poorer communities, there's less patient education and awareness
about how to safely research a cosmetic surgery procedure, which can
make people especially susceptible to con artists.

"It keeps happening in minority communities here in California. People
prey on their own people. They speak the same language and take
advantage of the patient's comfort level," says Dr. Wells.

Indiana plastic surgeon Dr. Barry L. Eppley says there's even a strategy
at play.

"Cosmetic charlatans like those two sisters set up shop for about six
months until something bad happens, and then they run. The trap is set
up from the beginning. They say the procedure is simple and offer a very
low cost, and then find people who will bypass anything for convenience
and price," says Eppley.

In a similar rash of cases this past March in New Jersey, six women
were hospitalized for severe reactions after receiving what was billed
as silicone injections to their buttocks. An investigation revealed that
they were actually injected with the same thick cement-like goop
contractors use to caulk bathroom tiles.

All six women received emergency surgery to remove the hardened caulking
from their bodies, as well a strong series of antibiotics. Doctors say
they're lucky to be alive.

So why do women keep getting talked into silicone injections? Dr. Eppley
says it's the allure of a permanent result.

"There's no such thing as a permanent injectable filler -- even your own
fat isn't 100% permanent. Silicone injections are not an FDA-approved
procedure anywhere on the body. The only people doing this aren't even
on the fringes of the medical world, they're straight-up black
marketers," says Dr. Eppley.

As for safe, medically accepted alternatives to enhancing the
buttocks, implants similar to breast implants and fat injections also
known as the "Brazilian Butt Lift" are today's standard.

Yet outside of Latin American and South American communities, tush
enhancements are not popular.

Butt augmentations are the third least common form of cosmetic surgery
in the United States, with only 3,900 performed in 2009 at a national
average cost of about $4,315, according to the ASPS.

Surgeons cite a high patient dissatisfaction rate due to wear and tear
that comes with continually sitting on the implants. Volume loss over
time from fat injections as they re-absorb into the body also leaves
many patients unhappy.

Yet the desire for other cosmetic procedures like breast augmentation
and botox continues to grow in spite of the recession.

Women in search of a sexier figure or younger complexion need to
always take the time to do their research, says Dr. Eppley.

"Don't ever put convenience and cost before hiring a fully accredited
surgeon and choosing a procedure that is FDA-approved and well within
the standard level of care. You could pay for it with your life."

Dr. Persky was privileged to attend and honored to speak at The 6'th Annual Facial Cosmetic Surgery 2010 meeting, an international multi-specialty symposium which gathered over 800 core facial plastic surgery specialists from 23 countries. World leaders in the specialties of Cosmetic Dermatology, Facial Plastic Surgery, Oculoplastic Surgery, and Plastic Surgery gathered in the Las Vegas mid summer desert heat to discuss "hot" topics in their fields.

Dr. Persky shared his nearly 3 year experience with FRACTIONATED CO2
LASER skin rejuvenation (Fraxel repair). Fractionated CO2 laser
treatment has become an accepted non-surgical technique of facial
rejuvenation.

Dermatology experts Susan Weinkle, MD; Ava Shamban, MD; Wendy
Roberts, MD; Hema Sundaram, MD; Phill Werchler, MD; Rebecca Fitzgerald,
MD; Mary Lupo, MD; David Goldberg, MD; and Lisa Grunebaum, MD to name a
few shared their skin care "pearls" as well as their injection and other
rejuvenation techniques.

Every patient should be on a RETINOID (Retinol, Retin-A, Renova,
etc.)at night if tolerated for optimal skin tone, color, and texture.
Fillers are very popular. Dr. Persky believes that SCULPTRA will become
a very popular filler in the next five years. TAZORAC is a wonderful
topical for the treatment of acne and pore size.

Non-invasive skin tightening technologies were discussed. Zeltiq
looks promising using cold to shrink the fat of love handles and
abdominal pooches. Ulthera uses ultra-sound energy to lift the tissues
of the forehead, neck, and face. Pelleve` uses radiofrequency (RF)
energy for moderate collagen stimulation and lifting of the face.
Primaevia uses bipolar radiofrequency to diminish wrinkles. E-Matrix
uses sublative RF to rejuvenate darker skin types.

Facelift experts are using fat transfer in nearly all of their lifts
to supplement facial volume. Fat transfer to the face is alive and
well! The latest Rhinoplasty and Blepharoplasty techniques were shared
by world leaders as well.

There are new filling agents that will soon compete with hyaluronic
acids such as Juvederm and Restylane. The topical "Botox" Revance looks
to be a few years away from hitting the market with promising results
on the treatment of "crow's feet" and excessive sweating of the
underarms and hands.

That's all the news from Las Vegas, where all of the faculty were
great, all of the exhibitors friendly, and all of the children not
allowed in the casino (with apologies to Garrison Keillor).

Dr. Persky will be speaking about the use of Fraxel repair in the
treatment of acne scarring and skin rejuvenation at the American Academy
of Facial Plastic and Reconstructive Surgery Annual Meeting in Boston
this September. Until the next update, have a great summer, wear your
sunscreen, and be well.
Having participated in answering facial plastic surgery questions on line for the past few years, I must say that some of our patients ask the "darndest things"! I have decided to share some of those not so frequently asked questions
with my friends here. In the future I will make sincere attempts to
limit the entries in this blog catagory to only those LOL questions and
answers. Enjoy:

Can i have lots of sex after 2 months rhinoplasty ?

facial plastic surgeon is a member of the American Academy of Facial
Plastic and Reconstructive Surgery (AAFPRS), the world's largest
association of facial plastic and reconstructive surgeons.

Hola,

You also earlier asked about your asymmetrical nose one month after
your rhinoplasty. We ask our rhinoplasty patients to avoid full contact
sports for at least 3 weeks after rhinoplasty. Those that are playing
sports that involve balls are told to be careful not to be hit in their
nose with the ball. In your case, it is okay to "have lot's of sex" 2
months after your rhinoplasty, but once again, try to keep the balls
from hitting your nose. Good luck on all fronts.

"What shall it profit a man if he gains the whole world but loses his soul" -Jesus

When evaluating new and exciting medical technologies and advances it is essential that we use good science as well as common sense in our evaluations. Unfortunately many highly touted technologies are just that, hype and company generated promotion without proven science or clinical studies to substantiate their claims.
I have just returned from the Facial Cosmetic Surgery 2010 meeting, a
meeting well respected for its faculty and its content. Sadly, even at
great scientific meetings, valuable time and mental effort is sometimes
wasted in separating the truth from the hype.

I have spent the 30 years of my professional life in medicine
establishing my reputation, and am not willing to compromise ethical
care to my patients based on unethical paid consultant presentations. I
will continue to offer my patients only clinically proven, safe, and
effective treatments in a caring manner. After all, we should do onto
to others as we wish they do unto us.

Be well.

The Beatles weren't the only ones to ponder that question. Well, a website called Make Me Old (http://www.makeold.com) will do the aging for you.
What is striking is the loss of volume in the face as it ages.
Fortunately, in real life, we can use fillers such as Sculptra, Radiesse, Restylane, Juvederm, and fat to replace volume in the face.
The trick is finding the best physician "sculpturer" to accomplish the
rejuvenation. Remember, the best plastic surgery is always natural and
undectable to fellow shoppers on Rodeo Drive in Beverly Hills. It is
okay to "puff" the face naturally, but avoid those Pillsbury Boy
aesthetic physicians. Choose your facial cosmetic physician most
carefully. Be well.
It has been a while since I last wrote; hope that your summer is going along well.
We traveled to Italy, visiting the Vatican and Michaelango's Sistine Chapel, Bernini's sculptures, and Da Vinci's masterpieces.

Michelangelo’s David (1504) in the Uffizi Museum, wow! Wonder what
these great masters would be doing if they were facial plastic surgeons
today?

They probably would have been in Las Vegas (110 degrees) last week
at the Bellagio attending Facial Cosmetic Surgery 2010, a
multi-disciplininary meeting of some of today’s master artists in
plastic surgery. Many of the world's top leaders in plastic surgery,
facial plastic surgery, ophthalmic-plastic surgery, and dermatology
shared their knowledge and experience at an incredible meeting.

One of the "hot" topics (full pun intended) this year was
fractionated CO2 laser skin rejuvenation. I was privileged to present
my nearly 3 year experience with Fraxel repair fractionated CO2 laser.
Course chairman Randy Waldman, MD, Ed Cortez, MD, and Waffles Wu, MD put
together a faculty that would have kept the great Italian Masters awake
with interest (maybe with a few cups of coffee) with their incredible
results.

I look forward to sharing what's new in facial plastic surgery with
you in the near future, unable to right now as I am preparing for
tomorrow's rhinoplasty surgery on my patient, oh, his name?... David.
Be well , ciao.
