An article on Nov. 12, 2014 in MStars News, entitled “Mary Kate Olsen: Plastic Surgery or Pregnancy? What Happened To The Child Star’s Face?” asks what we think Mary Kate Olsen has done to appear so different from her identical twin sister Ashely Olsen. It is obvious that Mary Kate has received facial filler treatment that has not been “identically” received by Ashley.

When one twin drastically appears different from the other, it is rather easy to compare and contrast the differences in their faces. This photo shows the obvious filler in Mary Kate’s lips, her cheeks, temples, forehead, and jawline resulting in a more masculine appearance than her twin sister. The most likely filler used, in my opinion, is Voluma, a great deal of Voluma. In a 28 year old woman, we may use 1-2 syringes of Voluma for subtle volumization of the cheeks. To create the changes seen in Ms. Olsen, 28 years old, would likely take at least 6 to 10 syringes of Voluma. Other fillers were likely used including Juevederm in her lips, and perhaps Sculptra in her face.

The MStars article postulates that Mary Kate’s facial changes may be due to pregnancy. Though pregnancy can add volume to the face, it does not add it in the manner Ms. Olsen’s face has changed.

Ironically, for all of the “rejuvenation” work that Mary Kate has had done, she now appears at least 10 years older than her twin. We have seen this time and again when young patients receive excess filler. They look much older, and the same physicians that create this look, use the fillers in a similar manner attempting to make 50 and 60 year old women appear 40-ish. In my opinion, Mary Kate now looks like an older woman who is trying to look younger.

So, what happens when one twin has aggressive facial volumization and the other does not? Well, the obvious plastic surgery changes are more obvious, but at least for someone who has difficulty for 25 years telling them apart, I can finally differentiate Ashley from Mary Kate. Choose your facial plastic surgeon most carefully, based on their skill, experience, and most importantly, their natural artistic vision. Be well.

Using 3 of Allergan’s products on our patients who wish to look their best is giving an amazing immediate transformation, taking years of aging away in minutes. We begin “The Allergan Facial Rejuvenation” with a detailed facial analysis, evaluating where the individual can best use volume to support the rest of the face. It is helpful to have a photo of the individual when they were in their mid 20s to 30s to have something to compare.

Voluma ba2Voluma is used first to restore volume to the cheeks, often resulting in improvement of the lower eye lid hollow, the nasolabial fold, the vertical mid-cheek lines, and the jawline. Second we use Juvederm XC Plus to re-inflate the lips and correct the dreaded smoker’s lines around the mouth. A small amount of Juvederm can be used in the nasolabial folds (laugh lines) if needed. The third product that we use is Botox to treat the “11” lines of the forehead, to lift the eyebrows, treat the crow’s feet, and any other areas that would benefit from it’s use.

The Allergan Facial Rejuvenation results in an instantaneous natural youthful apppearance in the hands of Drs. Persky and Sunder. Feel free to contact us with any questions that you may have regarding facial cosmetic surgery. Be well.

Michael A. Persky, MD, Mauricio de Maio, MD @ de Maio Voluma Masters Technique Course

Michael A. Persky, MD, Mauricio de Maio, MD @ de Maio Voluma Masters Technique Course

Voluma (Allergan, Irvine,CA) is a revolutionary facial filler that has just been FDA approved for use in the U.S. Voluma has been used outside of the US for many years. Voluma is the longest lasting hyaluronic acid filler, lasting up to 2 years. It is an enhanced form of Juvederm, an injectable filler that we have been using for years.

The most common and pressing question about Voluma is, “How much does Voluma cost?” The answer to this question is complex. With injectable fillers, patients must realize that they are not paying for just a product, Voluma in this case, but they are paying for our physicians expertise, experience, and artistic vision of beauty, all of which result in natural long lasting results with the assurance of patient safety being a priority. Our knowledge of anatomy, where in the face a millimeter can be the difference between an excellent result or a serious untoward complication, gives our patients the best chance to avoid adverse events. All of our injection treatments are performed by our surgeons. At Persky

Voluma 2Sunder Facial Plastic Surgery the cost of Voluma varies depending on how much the patient needs. Both Dr. Persky and Dr. Sunder are Master Injectors of Voluma, having completed the de Maio Master’s Voluma Technique Course taught by the brillant Mauricio de Maio, MD, Sao Paulo, Brazil, a renowned plastic surgeon and expert Voluma pioneer injector. Dr. Sunder and Dr. Persky were privileged to be included in the group of only 30 US facial filler injectors to be recently trained by Dr. de Maio.

Voluma Persky injectingThe FDA approval studies on Voluma concluding that Voluma lasts up to 2 years was based on the trial US physicians injecting an average of 5 to 6 syringes of Voluma. 5 to 6 vials of Voluma is a very expensive proposition. Dr. Persky and Dr. Sunder (as well as our patients) have benefited from de Maio’s Advanced Master techniques by becoming experts in where to inject Voluma to get the “biggest bang for the buck.” Dr. de Maio, and now Dr. Persky and Dr. Sunder are consistently able to get the same or even better facial aesthetic rejuvenation with 1 to 2 syringes of Voluma, rather than having to use 5 to 6 syringes. Therefore our patients benefit not only from a natural facial rejuvenation of their cheeks, eyes, nasolabial folds, marionette lines, jowls, and jawlines, but they are able to achieve those results with less expense.

Voluma before-after-juvederm-voluma-1In order to achieve these dramatic results with less Voluma, all patients undergo a comprehensive beauty analysis by our physicians. It is the beauty evaluation, technical injection skills, and most importantly artistic vision of Drs. Persky and Sunder that results in very high patient satisfaction and natural aesthetic results.

The cost of Voluma at Persky Sunder Facial Plastic Surgery varies from $1200 to $800 per syringe depending upon how much volume the patient needs over a year’s time. The first syringe of Voluma is $1200, each syringe used at the same injection session is $50 to $100 less.

voluma-before-and-after-2Voluma lasts up to 2 years. As with all aesthetic procedures involving the face, it is most important to choose your physician most carefully. There will be plenty of clinics offering Voluma at discounted prices, injected by non-physicians and even untrained, inexperienced physicians, both of whom may lack the artistic vision to insure excellent, natural results. Unfortunately we will be seeing more “Housewives of Beverly Hills, Housewives of Miami, etc.” cheeks and lips on a stick unnatural results from improper use of Voluma. Buyer beware!

To learn more about Voluma, feel free to contact us, watch our videos, and follow our blogs. We look forward to the privilege of helping you achieve your facial aesthetic goals! Be well.

“There is no such thing as an ugly woman, only a man with a utter lack of imagination!” Anonymous

In a recent online article Grated listed it’s 10 most unattractive (ugly is an ugly word!) celebrities. After looking at the photos, we must admit that many on the list have been previously referred to during consultations here at Persky Sunder Facial Plastic Surgery by patients who, “…do not want to look like so and so!” We have undertaken the task of reviewing each of the listed celebrities photos, pointing out what exactly it is that has landed them on this unenviable list.

Before going any farther, let us remind ourselves that we are only refering to superficial, outer appearance, and that in the celebrity world there are many externally “beautiful” people not on this list who are “ugly to the bone” on the inside. Conversely, in defense of those on this list, yes, they may have made some bad plastic surgical physician choices in the past, but they may be beautiful people on the inside, where beauty really counts.

We must remember that it only takes millimeters of change one way or the other for the human primordial brain within fractions of a second to determine if a face is symmetric, healthy, and one that another human would wish to share genes with (reproduce) for survival of the species. When the millimeters of change are positive, it is rejuvenative and youthful. On the other hand, as in the case with many of the celebs on this list, when the millimeters of change are asymmetric, or measured in centimeters (or seemingly feet), then the unnatural change can cause the face to look like a genetic mutant (not to infer anything about the celebs on Grated’s top 10 list).

With that in mind and with tongue in cheek, here we go:

10. Melanie Griffith has had her share of bad plastic surgery and fillers over the years since she first met Miami Vice’s Don Johnson, but actually we feel that she is returning to normalacy (thank you Antonio Banderas), and most definitely there are other celebs out there more deserving to be on this list (see below). Melanie’s lips had been filled with something permanent, which created an imbalance, her upper lip being much larger than her lower lip. The proper lip proportions have the upper being smaller than the lower (upper lip should be ~70% the size of the lower lip).

9. Janice Dickinson has lips and breasts that enter the room a time zone ahead of the rest of her body. Other than her alien sized lips, her facial volume otherwise looks good for an older woman.

8. Jackie Stallone appears to have work done on her face by the same surgeon who worked on her son, Sly. Whenever it is obvious that someone has had plastic surgery, the surgeon has failed the patient (see the late Michael Jackson). Ms. Stallone has excessive lip volume, facial volume, elevated eyebrows which all add up to make her look like she is Carrot Top’s mother. By the way, Carrot Top’s plastic surgery is most deserving of a spot on this list.

7. Mickey Rourke has been a veteran of these lists for many years after having a premature, poorly performed facelift many years ago. Since then, boxing and multiple surgeries have changed Mickey’s appearance. His current appearance is actually becoming more normal, so this may be one of his final appearances on lists such as these, as long as he doesn’t go back to the same surgeon for a revision.

6. Male rocker Pete Burns has been transformed to a large “lips and cheeks on a stick” female look a like (see Housewives of Beverly Hills, he would fit right in). What is it about those lips? We are constantly having to reassure our prospective patients that it is not the injectable filler treatment that causes these abberations, but rather the injecting physician or nurse’s lack of artistic vision. By the way Pete, I hope that your coat is imitation, or that you are staying far away from PETA.

5. Cher is one of our favs and personally we don’t beleive that she belongs on this list. Cher has maintained her facial volume relatively well, and takes excellent care of her skin with non-invasive stimulating treatments. She would likely be a hugh fan of Ultherapy by Ulthera, lifting and tightening of the face, neck, and brows. “I’ve Got You Babe!”

4. David Gest may have the gold and platinum albums behind him, is worth multi-millions, but like his late friend Michael Jackson is the two word answer to a RealSelf.com question that we responded to a few years ago. The question was, “Do celebrities get better plastic surgery because they have more money?” Only the ones who choose their surgeons most carefully! Mr. Gest’s exaggerated elevation of his forehead and eyebrows, along with the feminizing upper eye lid blepharoplasty (can you say Bruce Jenner Kardashian?), the lower eyelid sceral show and ectropion from poorly performed blepharoplasty, and disproportionate volumization of his face have landed him most deservedly on this list of celebs.

3. Poor Donatella Versace, don’t know who she saw for her plastic surgery, but they have turned her into an aging Willie Mays (sorry Willie). Her facial volumization with fillers and/or fat is exactly what keeps many patients away from ever stepping inside a plastic surgeon’s office…”fat, pudgy face”.

2. Jodie Marsh. Not sure who she is, but what is again quite obvious is that she has had plastic surgery that is not in line with Leonardo da Vinci’s Devine Ratio of proportion. All that you can say about Jodie’s plastic surgery is that they tried their “breast”!

Drum roll please……..1. Iggy Pop (Really?) Mr. Pop is an example of where loss of facial volume and not dressing appropriately for one’s age will land you, as the winner of Grate’s Ugliest Person List! As we age, we lose facial volume, mainly fat and bone, but also some muscle. Before the days of filler and Ulthera, Catherine Deneuve said, “At a certain point in a woman’s life she must choose between her face or her fanny!” Fortunately today with fillers such as Sculptra, Radiesse, Juvederm, Restylane, and others, a woman (and aging rock stars) can remain slim but still maintain their facial volume.

Honorable Mention: Mick Jagger, the great front man of the Rolling Stones. See Iggy Pop above.

Be well!

It’s always nice to start a Monday morning on a positive note. We received this email this morning from a patient whom we have been treating for years with BOTOX® Cosmetic and fillers, but she was always very reluctant to do anything with her lips for fear of having that “obvious” lip augmentation appearance.

Woman with full, voluptuous lips against a black background.

Last week, after closely examining our office staff members whose lips we have treated, she agreed to a conservative lip augmentation injection with JUVEDERM®. We used a cannula to inject a conservative amount of JUVEDERM® a few days ago. As with anything in plastic surgery, less is usually more, one should not have the obvious “plastic look”, especially with lips. After her treatment, when given the mirror to look at her lips, our patient said, “Oh, my daughter is going to know!” We asked that she call us and let us know.

Here is her email today: ” Hi Dr. Persky,

Just wanted to tell you I love my lips and my daughter did not notice until I told her. I could probably even add a little more to my upper lip but I’m hesitant to do it.

And…I got carded – twice – Saturday night thanks to you!

Thanks for everything,”
Be well.

“A man had given all other bliss, And all his worldly worth for this, To waste his whole heart in one kiss Upon her perfect lips” Alfred, Lord Tennyson 1809-1892

Our first patient of the day today was reading British author E. L. James erotic novel “Fifty Shades of Grey” when I entered the treatment room. 50 Shades is not exactly the “best written” book in the history of literature, and probably has Lord Tennyson turning in his grave with each page read. As they say about 50 Shades, “the writing is not that great, but the story kept my attention.” One of the many oft repeated phrases by Christian to Annatasia in the novel is, “I told you to stop biting your lip, you know that it drives me crazy!”

Sexy lips not only drive the main character of 50 Shades crazy, but they are quickly becoming one of the most requested treatments in our office. Lips do not, and should not be enormous to be sexy. In fact, less is usually more when it comes to natural lips. New techniques and injectable products have revolutionized lip augmentation in our practice.

Like snowflakes, no two sets of lips are identical. The first step in creating beautiful sexy lips is analyzing the patient’s lips with an artistic eye. Secondly, we then visualize what the ideal lip would be for that individual. Leonardo da Vinci’s Devine Proportion or the Phi Ratio of 1:1.67 is kept in mind when creating sexy lips. The lower lip should be larger than the upper lip by close to that golden ratio (upper lip should be 70% the size of the lower lip). Lastly, the lips must be technically injected properly with the appropriate injectable product(s).

sharpvsmicrocannula_620pxThe introduction and use of blunt tipped canullas have revolutionized our lip treatments. The blunt tip has the advantage of not cutting through nerves and blood vessels of the lips so that the injections are much less discomforting, and there is considerably less, if any, bruising. Upon patient request, we can use local anesthesia nerve blocks prior to lip injections, but with the use of canullas and local anesthetic in the fillers, most patients do not require the additional anesthesia. The blunt tipped canulla also finds its way into distinct anatomic spaces of the lip that have lost fat, allowing the spaces to be easily refilled in a natural way. This results in shortening, elevation, and eversion of the upper lip even before any filler has been placed into the body of the lip. Juvederm is used to replace lost fat deep in the lip, as well as in the body of the lip to increase volume. Restylane is used to accentuate the vermillion border (the red-white border) of the lips. The newest FDA approved filler, Bolotero, is a thin hyaluronic acid that is used for the fine vertical lines of the lips.

Seeing sexy new natural lips on our patient this morning after her 15 minute treatment session, the comfort that she underwent the procedure with, and her copy of “Fifty Shades of Grey”, I realized that thanks to these new techniques and products, our treatment room no longer rivals Christian’s sado-masochistic, tortuous “Red Room” in painful experiences, but just in case, for those patients who request it, we will give them a “safe word” during their injection treatment.

A foolish man tells a woman to stop talking, but a wise man tells her that her mouth is extremely beautiful when her lips are closed” Anonymous (for obvious reasons)

Wishing you happiness and health this holiday season and into the New Year. Be well!
