Happy and healthy New Year, we hope that your 2017 is off to a great start!

We are most excited to share what’s new at our practice since January 1.  We have added 3-dimensional photography, LED light therapy, and two minimally invasive, non-surgical state of the art, safe and effective treatments that lift and tighten your face.  Additionally, we have added “The Plan”, a comprehensive natural beauty long term customized plan to help you achieve and maintain your aesthetic goals.

Dr Persky modelIf you ever have watched the HBO show “Westworld” and wondered about the technology behind creation of the robots faces, perhaps 3-D photography has something to do with it.  Our brand new VECTRA  3-Dimensional Camera system takes three facial photos that are automatically stitched into one 3D image with VECTRA software.  We can evaluate facial contours without color distraction (ever wonder what you would look like as a statue?), the gray mode reveals contours, presenting opportunities for corrective procedures. VECTRA allows quantification of subsurface skin conditions, giving you an immediate  sun damage “report card.”

Canfield’s proprietary RBX® technology separates the unique color signatures of red and brown skin components for unequaled visualization of skin conditions. Both prior to and after facial treatments that affect facial volume, we are capable of seeing an automatic volume difference measurement with one click.

Automated measurements allow for quick and accurate computer imaging (rhinoplasty, fillers, chin implant, lasers, etc.) only possible with VECTRA 3D technology, helping our patients understand their current condition and what they may look like after their treatment.
We can visualize and discuss your aesthetic hopes and expectations, quickly arriving at a shared understanding of objectives. VECTRA allows demonstration of the difference between the likely outcome with only one or multiple procedures. With Face Sculptor we can explore the possibilities and help you decide on the best possible treatment plan.  When we’re finished, we can give you a printed plan including selectable VECTRA2D views of pre-op and/or simulation.

At Thanksgiving dinner an esthetician friend shared her experience with LED light therapy, the top of the line Celluma system.  I see her and her mother once a year and immediately noticed the improvement in her mother’s skin.  They shared that Celluma treatments alone were the reason for her improved skin tone, texture, color, and overall aging.  Celluma is a relaxing, comforting, 30 minute two to three times a week LED light therapy that helps with acne, wrinkles, and even treats soreness and pain anywhere on the body.

For the past twenty years “thread lifts” with permanent sutures have come and gone.  Dr. Persky never did these permanent thread lifts because of the risk of permanent problems, asymmetry, short term results, and frequently unnatural appearance.  Fortunately for you, there is a new kid on the block of thread lifting.  Silhouette Instalift is a non-surgical minimal invasive new technology that allows us to use our natural artistic vision to immediately lift the face, jowls, and neck, as well lift to the cheeks .  Silhouette Instalift threads (or sutures) are made of an absorbable lactic acid material (similar what the bio-stimulatory injectable filler Sculptra is made of) that adds needed volume to the face.  There are not any permanent problems with permanent sutures.  Our patients have been thrilled with their immediate results, ease and comfort of the treatment, minimal downtime, and their natural more youthful appearance.Dr Persky model

, to our practice.  Now you can truly “Choose” to age gracefully with natural facial, neck, and body rejuvenation.  Profound uses the incredible advanced technology of pairs of micro-needles which penetrate the skin 1 to 2 mm heating the tissue to 67 degrees C for 3 seconds with radio frequency energy controlled by temperature sensors at the tip of each micro-needle, insuring safety and maximum stimulation of new collagen and elastin in the skin.  Profound  Dr Persky model

is performed by our surgeons after the face is anesthesized first with topical anesthesia then with local anesthetic nerve blocks.  Once the face is numbed, the treatment is very comfortable taking about an hour to complete.  Nearly 100% of patients see a notable difference within 2 to 3 months with continued improvement for 6 to 9 months.  Profound is the closest thing that you will get to a facelift with a non-surgical procedure. It is positioned in our practice between Ultherapy (for those patients who want lifting without downtime) and surgical face/neck lifting.  The only downside to Profound is that there is a 5 to 7 day period of swelling and bruising downtime.

Lastly, we believe that our patients deserve to look their natural best for the rest of their lives.  We now have “Your Natural Beauty Plan” or just “The Plan” for short.  Each patient is given an individualized short and long term beauty plan to achieve and maintain their rejuvenation and natural beauty for many years to come.

The three comments that I often hear both in the office and socially are:

“Dr. Persky,can you tell when someone has had some plastic surgery done?”

 “Doc, why does everyone who has plastic surgery look the same?”  

“I want to look better, but I don’t want those big apple cheeks and hugh lips!”

We all can tell when someone has had obvious plastic surgery.  If our treatments result in a obvious “done” result, then we have failed as plastic surgeons.  These obvious  cases of poorly performed aesthetic treatments without a sense of artistic vision may “all look the same.”  They have the same big lips, big cheeks, and paralyzed facial expressions.

Conversely, when plastic surgery or aesthetic treatments  are performed properly resulting in a natural appearance, then most of us, including myself and other expert plastic surgeons cannot tell that anything has been done.

The way to achieve your goal of looking better, a better more rested youthful version of yourself, is to make sure that your aesthetic physician is well trained, skilled, knowledgable, and most importantly posesses a natural sense of artistic vision.

Beauty is perceived and appreciated in a blink of the eye, in a millisecond.  Our brains are programmed to see natural proportions, symmetry, light, shadows, and highlights and immediately tell us if someone (Jennifer Lopez) or something (say a Ferrari) is beautiful and proportioned.  On the other hand, if a face is disproportionate, our subconscious primordial brain immediately tells us that the appearance is unhealthy, infertile, and someone with whom we should not exchange DNA with for the sake of our species ultimate survival; they look like freaky aliens.   All the while, our cognitive or verbal response is often, “She/he looks ‘funny’.  What did they have done?”

At your initial consultation as well as at each of your subsequent visits, Dr. Persky uses his expert sense of artistic vision to analyze your face.  This analysis allows Dr. Persky to give you your blueprint for natural rejuvenation.  The analysis includes your forehead, eyebrows, eyes,  nose, mouth, neck, as well as your skin tone, texture, and color.  Your overall facial shape and natural Ogee curves are considered as well.

 

The face can be divided into thirds horizontally (the forehead, the mid-face, and the mouth) and vertical fifths, each the width of the distance between the inner corners of the eyes (the only measurement on the face that does not change with aging).   Leonardo da Vinci described the “Devine Golden Ratio” or “Phi” over 500 years ago and it is as relevant today as it was when the Mona Lisa was painted (did you know that da Vinci never signed his works as he believed that a work of art was never complete!)  Dr. Persky uses a Phi caliper to measure the distance between your eyes, and uses that and his built-in artistic sense of proportions as a guide to restoring your face to it’s natural appearance before the aging process of volume loss of bone and fat began (unfortunately it begins in all of us from about age 25 on).

Congratulations to Sandra Bullock, People Magazine’s 2015 Most Beautiful Woman. The common feature of the beautiful woman featured in “The
World’s Most Beautiful issue (#MostBeautiful) is the exquisite symmetrical geometric proportions of their faces. Leonardo da Vinci used mathematics and observation dividing the face into horizontal thirds and vertical fifths (each fifth the width of the distance between the inter corners of the eyes.) da Vinci(1485) discovered the “Devine” ratio of 1 to 1.68 representing beauty. For instance, if the upper lip is 1 unit wide, a beautiful natural lower lip should be 1.68 units wide. The same holds for the width of a nose to the length of it.

We meet new patients everyday who request an honest evaluation of their facial appearance. Fortunately, both Dr. Sarmela Sunder and Dr. Michael Persky are experts trained in facial proportions and facial anatomy. We use the classic golden proportions first used by da Vinci in his art to guide our facial aesthetic procedures, whether with injectable fillers or surgical procedures.

We take our time to evaluate the patient’s anatomy, listen to their wishes, get to know what their motivation is for their treatment wishes, and establish a relationship and plan for their optimum treatment. Men who want to look their best in a natural way are turning up at our office for Voluma treatment. Both Sarmela Sunder, MD and Michael Persky, MD are de Maio Master Injection Technique Voluma trained. We share a natural artistic vision with our patients. The artistic aesthetic for men is different. Drs. Persky and Sunder respect the gender difference creating masculine beauty in men.

Academy Award Best Supporting Actor Jered Leto and Best Actor Matthew McConaughey stars of “The Dallas Buyers Club” are an excellent examples of the relatively straight, strong masculine angles of the natural male cheek. Gwyneth Paltrow has the fuller and more curved natural feminine cheek of the female.

With just 1 to 2 syringes of Voluma in most of our patients, the hands of time are turned back in fifteen minutes. The treatment is quick with minimal discomfort as our nurse simultaneously touches the patients skin with ice as Voluma is placed. There is the possibility of swelling for 2 to 3 days, and if bruising occurs it may take 3 to 5 days to resolve (we treat patients a very small minority of patients who bruise with electro-magetic field energy which helps speed resolution). We like to see our patients for a “face check” 7 to 10 days after their Voluma treatment to make sure the product, which lasts up to 2 years, is smooth and that the patients face is symmetric. Often times patients request more volume at this visit, if appropriate a small amount can be added. One of the benefits of conservative Voluma treatment is that it can be “tweaked” in small increments.

The most important aspect of Voluma treatment from a patient standpoint is to make sure to choose your injecting physician most carefully to insure beautiful aesthetically pleasing results in the safest, most efficient and pain free manner.

Couldn’t help but be distracted by TV on in gym this morning with “Housewives of Miami”. I had to watch to examine what made the characters on the screen appear so unnatural. Whoever is doing this work would be well advised to stop giving facial aesthetics a bad name. Seeing these women reinforces 3 things that excellent aesthetic physicians live by: 1) If it is obvious that something has been done to a patient, then we as aesthetic physicians have failed our patient. 2) Less is more! 3) Without artistic vision one can have technical success with cosmetic treatments, yet aesthetic failure!
